As diabetes technology is becoming commonplace in our practice, figuring out how to make sense of all the data can seem overwhelming. Join Diana Issacs and Coach Beverly for a truly unique learning experience. Dr. Isaacs has a special knack for breaking down the essential elements of the Ambulatory Glucose Profile (AGP) report to provide participants with a clear road map for data interpretation. She includes many sample practice cases utilizing CGM data for various types of people with diabetes including type 2 and people with type 1 not on pumps.  Coach Beverly will build on Dr. Isaacss' presentation and switch the focus to the person living with diabetes. Using a case study approach, she will provide strategies to integrate the AGP with person-centered care that empowers individuals to experience increased confidence in their diabetes self-management.

By attending this interactive workshop, participants will become more confident in interpreting the AGP and continuous glucose monitor (CGM) data and determining needed medication and lifestyle adjustments with a person-centered approach.

      This session provides healthcare providers with actionable strategies to integrate weight-inclusive care into diabetes management practices, focusing on improving outcomes through respect for body diversity and individual autonomy. Weight stigma is a well-documented barrier to effective care, contributing to poorer glycemic control, disordered eating, and care avoidance. By shifting from a weight-focused approach to one aligned with Health at Every Size (HAES®), providers can foster trust, enhance participant engagement, and support sustainable health behaviors.

      Jessica Jones, MS, RD, CDCES, will share evidence-based insights on mitigating weight stigma and applying HAES-aligned strategies in practice. Participants will learn how to build rapport, set realistic nutrition goals, and implement interventions that prioritize blood sugar management, joyful movement, and intuitive eating. This session equips providers with tools to deliver compassionate, person-centered care, improving both clinical outcomes and the provider-participant relationship.

        New: Assessing & Preventing Lower Extremity Ulcerations & Amputations

        Instructor:

        In this course, Dr. Mark Hinkes will describe the critical role of health care professionals and diabetes educators in the assessment of the lower extremities concerning diabetes foot ulcer and amputation prevention. He will outline the identification of risk factors for the development of diabetes foot ulcers and amputations and detail the steps involved in a comprehensive diabetes foot exam. Attendees will learn how to use international guidelines to stratify individuals into risk and screening categories combined with evidence-based medicine to prevent lower extremity complications.

        Objectives:

        1. Perform a comprehensive diabetes foot exam that includes a vascular and neurological evaluation plus identification of bone, soft tissue, and nail deformities.
        2. Utilize the information from the exam as data to stratify individuals into internationally accepted risk groups and refer to members of the interdisciplinary team for risk mitigation.
        3. Identify when to re-screen individuals depending upon identified risks and risk categories and apply the guidelines of the International Working Group on the Diabetic Foot (IWGDF) to prevent "category creep" leading to diabetes foot ulcers and amputations.
        4. Describe how risk data may be used to nourish machine learning and artificial intelligence tools to provide predictive analytics leading to personalized medicine.

        Learning Outcome:

        Healthcare professionals will gain confidence in assessment, screening, and risk mitigation to prevent lower extremity complications in individuals living with diabetes.

            Adverse childhood experiences (ACE) are associated with an increased risk of diabetes, heart disease, cancer & a variety of other health consequences for adults. This session reviews how diabetes care & education specialists can provide screening, assessment, & trauma-informed care to individuals who experienced ACEs & are living with toxic stress. We will explore strategies to address ACES & improve outcomes for individuals & communities. Throughout, we will focus on supporting self-care with a focus on recognizing & promoting resilience.

            Objectives:

            1. Discuss the benefits of assessing Adverse Childhood Experiences (ACE) in individuals with diabetes.
            2. State the relationship between ACE Scores & the risk of future health complications.
            3. Describe a person-centered approach to fostering resilience & self-care for individuals with toxic stress.
